Section A: Not Required

You are not required to complete Section A of this form.


Section B: Signatory Name

Complete the following mandatory fields in full. Enter your full name, exactly as it appears on your ID documents:


  • Title: (Dr, Fr, Mr., Ms., Mrs., Miss, Rev)
  • Surname
  • Given Names (First & Middle Names)


If you have ever changed your name or been known by another name, list your previous other names in the Full Name fields. This includes maiden names.

Tips

  • Enter your full legal name, exactly as it appears on your identification documents.
  • Do not include nicknames, aliases or shortened versions of your name

Section C: Signature

Tips

  • Please sign this section with a black pen
  • A physical “wet” signature is required
  • NAB will not accept electronic signatures

  • Why do I need to provide a Specimen Signature when I’ve already signed the Certificate of Identity?

    CDF partners with the National Australia Bank (NAB) to provide our clients with a NAB Cheque “Link” Account – this provides account holders with access to the NAB branch network and a unique BSB and account number for each CDF cheque account. 


    NAB requires all signatories to be fully identified, using a Certificate of Identity Form and provide a sample of their signature, using NAB's Specimen Signature Card.
